Methods Medline, Cochrane, EMBASE, and Google Scholar were searched for studies using perfusion MRI and/or MR spectroscopy published up to March 4, 2015 which differentiated between recurrent tumor vs. necrosis in Perfusion imaging can play an important role in determining the malignancy grade of a CNS tumor. Perfusion depends on the vascularity of a tumor and is not dependent on the breakdown of the blood-brain barrier. The amount of perfusion shows a better correlation with the grade of malignancy of a tumor than the amount of contrast enhancement. MR perfusion imaging is becoming an increasingly common means of evaluating a variety of cerebral pathologies, including tumors and ischemia. In particular, there has been great interest in the use of MR perfusion imaging for both assessing brain tumor grade and for monitoring for tumor recurrence in previously treated patients. Recent studies have indicated that perfusion MR imaging, both evaluation of relative CBV by using a DSC technique and CBF by using a PASL technique, can help predict the grade of astrocytoma and the likelihood of rapid progression.
